How do I register with THRIVE CARE?
STEP 1:
Enter your details in the Registration Form below. We’ll ask you to upload or email a copy of your NDIS plan
STEP 2:
We’ll use your plan to create a Service Agreement between you and Thrive Care for you to review and sign
STEP 3:
Let us know which service providers you want to continue using and send us your provider agreements with them
STEP 4:
We’ll send you your login details to access your personal online dashboard with all your plan and budget details
